HSG Medicated Uterine Film

Hysterosalpingography (abbreviated HSG), commonly known as medicated uterine and tubal imaging. It is performed to visualize the tubes of women who cannot get pregnant, to check the lining of the uterus if menstrual bleeding does not come or decreases in quantity after any intervention applied to the uterus (such as childbirth or curettage), to find out the cause in case of failure to form a healthy pregnancy and recurrent miscarriage, and to evaluate infertility. Among these, the most common reason for application is infertility diagnosis.

How to Take a Uterine Film

HSG, or medicated uterine film, is performed by a gynecologist and a radiology technician. HSG is performed through X-ray rays. Therefore, the patient must make sure that she is not pregnant or sexually protected before the procedure.

HSG is performed four to five days after the end of menstruation in order to prevent menstrual bleeding from affecting the imaging and to avoid adversely affecting pregnancy against the risk of possible pregnancy. A general gynecological examination is performed before the procedure, and if there are signs of inflammation or infection, treatment is administered first. If HSG is performed during an infection, the procedure is postponed until after treatment, as the infection may spread into the abdomen.

The uterine X-ray is taken in the X-ray room of hospitals or private imaging centers. The X-ray dose given is not high. During the HSG, an examination instrument (speculum) is placed in the cervix while the patient is in the gynecological examination position. A thin tubular instrument called a cannula is inserted through the cervix where the examination instrument is placed. Through the cannula, the fluid used in filming is injected into the patient's uterus.

The fluid increases the visibility of the uterus and tubes and facilitates diagnosis. X-rays are taken several times during this fluid transfer process. The equipment implanted in the patient is removed. After the instruments have been removed, one last X-ray is taken and the procedure is completed. Since we administer the medication quickly and under pressure during the uterine X-ray, any mucus plug that could cause obstruction in the tubes and prevent pregnancy is removed. Pregnancies can therefore occur in the first six months after the procedure. Thus, the medicated uterine film (HSG) provides both diagnosis and treatment.

Is it Painful to Have a Uterine Film?

There may be a slight amount of pain during the HSG. Although the amount of pain varies from person to person, some pain is felt due to contraction in the uterine wall, especially when the fluid is injected. If the pain threshold of the patient is low and the contractions in the uterine wall may adversely affect the quality of imaging, a mild dose of anesthesia, which we call sedation, can be applied to the patient before the procedure. In this way, the patient is prevented from feeling pain during the medicated uterine film.
